I just answered this with regard to Spain. For other cultures I cannot offer
the same insight.

Some cultures commonly use hyphenated names which include the maternal and
paternal names for offspring. Anyone on the list from such an area? Doesn't
Spain have one of those cultures?

If it isn't a middle or maiden name, what is it?  This gets even more tricky
with non-English names where "last" names can get really confusing.
