Okay, I am missing something here - probably obvious but I don't see it.

 I downloaded that zip and extracted it, read the document, tried to run
the form in the database.  I got a compile error - can't find the
project or library - with the "ReportName =" line highlighted.  The
document says the ReportName parameter has to be provided, but I don't
see anyplace to provide it.  What am I missing?  Thanks, Tina
