Just to make it more complicated, I think they need the electronic form of
the pdf to send back to the government agency in question.

Or - the crude old fashioned way
Print the PDF and then overprint that output with the data appropriately
spaced

As in mailmerge into a .doc that is mostly newlines, tabs and spaces

Does the client have the reader or the full-powered suite? You could export
the information to a delimited text file, if they know how to get a pdf to
import it.
