Sounds like you need to import all the objects in the db to a fresh, empty mdb file. I'd start by importing everything but the problem form (and also leave behind the MSYS* tables, Access will build them afresh). Then try importing the problem one. I guess that it will refuse to import as it has become corrupted. You will then have to rebuild it from scratch or import it from a backup copy of the application.
