[AccessD] Data-driven option buttons

A listbox would have worked and not been very complicated to write, but frankly I don't like the look of it, especially in this context. I think the dynamic option buttons look way slicker.

Personally I like the option button look for a survey/questionnaire so I'm
glad you have a solution, but just for the sake of exploring other options,
would a list box have worked?  It would show all of the options instead of
just one at a time like a combo box.
