[AccessD] OT: Where have all my outlook toolbars gone, long time passing, where have all my outlook toolbars gone, long time agooooooohhhhh

I built a customer toolbar for my blue frog macro, went to some effort to
make it pretty, and now it is gone.  The same thing happened on my wife's
computer, I built the toolbar, and it just "disappeared".  If I right-click
on the menu area, my custom toolbar is not in the list of toolbars that
exist so I can't just select it to make it re-appear.  

Any idea where they go?  Is there any "getting them back"?
