[AccessD] Filter By Form problems

I got some code from someplace that was alleged to handle the Filter-by-form
situation. For a while it seemed to work but then it went sideways and now I
have to figure out a solution to the problem. Does anyone have any code I
might try out to solve this, or alternatively to volunteer a second pair of
eyes to look at the current code and see what's going wrong?

(The "second pair of eyes" theory I mentioned has proven itself many times.
It often happens that a colleague can spot what is wrong in two minutes
while you have wasted two days puzzling over it. Exhibit A is this list.)
