[AccessD] Toolbar Question and Problem

I am working on a database I created a long time ago with a custom toolbar. Memory fails me here. How do I determine what each item does? How did I name these?

One of the buttons on the tool bar closes the current form. This works correctly on all forms except on one where it asks for input to another form that is not open. What problem should I be looking for?
