[AccessD] The dread 2501: The Close action was canceled error

A reader is trying to use some simple form and report closing code that I 
offered in a blog entry. He's getting that notorious 2501: The Close action 
was canceled error. I'm going to check the archives later, but if someone 
knows off the top of their head how to resolve this, please let me know. The 
truth is, I've seen you guys discuss it several times, but I don't recall 
the resolution.
