[AccessD] How to Handle Multiple People with the Same Name

We use first initial last name as a user ID.  If the user ID is
already used, we append a number. DANDERSON2 etc.

DANDERSON is end dated but still exists. If he returns he could be
reactivated or possibly become DANDERSON3
