[dba-SQLServer] Speed of bulk insert

Interestingly, the speed of import has either - jumped from ~1K records /
second to about 11K records / second, OR - I previously missed a decimal
place (though the times required earlier on were still quite large so I
don't think that's it).  In any event ATM I am getting a speed consistently
around 11K / second for a bulk insert operation.  That is a significant
event in my ability to expeditiously process 100 million records.
