[AccessD] Pivot table builder

I'm trying to create a pivot table form in Access and no matter how
simple I try to make it, I immediately get an error message in the
detail area that says "The query could not be processed. The data
provider didn't supply any further error information."

The database is Access MDB. The version is Access 2002 SP-1.

Can anyone who has a simple pivot table form built please send me a
zipped MDB so I can verify that I can at least open and use one? If so,
please send it directly to me, zipped up.
