[AccessD] Unbound text box format

Sounds like you have saved a default for text boxes with that Input mask and/or Format. 
Clear the properties from a newly dragged textbox.
Then with that textbox selected, got to Form Design, click on the bottom button beside the 
control selector and select "Set Control Defaults".
